Clean-up of information on transmitter pages

I have made some graphical and layout changes to the pages that show digital television masts, including new icons and ITV company logos.

I have finally removed the old pre-PHP5 code from UK Free TV, and improved the way that pages cache. The better caching should improve page loading speed. As part of this process I have made lots of changes that to help with readability on the transmitter information pages.

The "location toolbar" shown at the top of each page now has larger icons and descriptive text:



Under each map, there is now another "toolbar". This shows links to DAB, AM/FM transmitters, all transmitters in the TV region, the BBC and Channel 3 region. These were link text before, but now have explanatory icons too:



The "self help relays" now have a better format:



The "Channel 3" history now has a better layout, which includes logos for all the old companies serving the mast.



The "help text" under the "How the transmission frequencies change over time" section has a much clearer layout:



The "Comparison of old analogue and current digital signal levels" section has been compressed:



In addition, the Freeview/Transmitters/All Transmitters page now lists all the TV and radio transmitters in the UK. The old page (which showed switchover dates) has been moved to News/History/DSO Dates.
